Amplified Artists Website Checklist/Submission Form: Visual Artists Amplified Artists websites are free of charge to all artists from, living in or performing regularly in Allen, Barren, Butler, Edmonson, Logan, Hart, Metcalfe, Warren and Simpson Counties in Kentucky. Visual Artists include: painters, crafters, sculptures, cartoonists, photographers, tatoo artists, graphic designers etc. The network will be all inclusive and your sending needed information would aid tremendously with this endeavor. In order to make this process as efficient as possible we are providing you with this checklist of materials that are likely to be used in the creation of your website (typically seven pages) at http://AmplifiedArtists.Ky.net. This form maybe used to update/change or complete exisiting posted information also. This form will provide us with your basic information, however in order to construct your website materials such as photographs and recordings may be necessary. Following the form is a checklist of the kinds of information you may want to submit - provide all materials mentiond in a single mailing if possible to Amplified Artists Promotions, 339 Phelps Way, Bowling Green, Ky 42104. Do not send originals or negatives. No materials will be returned. If you are more than on type of artist, complete forms for each type. ***If you already have a website it will be linked on the main page and designated as your "official website", however the webpages for this archive will still be constructed.

In addition to the above information the following will be necessary to have a more extensive, detailed website:

1. Pictures of yourself. On the back of each include a caption if desired and any photographer's credit. Include a hardcopy of your logo if applicable.

2. You may wish to send a more detailed lengthier biography describing the your accomplishments.

3.You may wish to attach a list and description of awards or honors you have received. 4. You may wish to attach a list of exhibits with brief description or pictures of each. 5. You may have three gallery pages with approximately seven full color pictures of your work. You can name these pages seperately if you wish ie watercolors, oils, pen and inks etc. Please include the name of the work on the back of the picture and any description you desire.

6. You may include pictures, pricing and ordering information for other merchandise such as post cards, t-shirts, books or newletters.

7. Attach a list of any websites you would liked linked from your site, if the list is long you may want to break it into categories. You may include brief descriptions.
